NKCC2 (Na⁺-K⁺-2Cl⁻ cotransporter 2), encoded by SLC12A1, is a renal-specific transporter located in the thick ascending limb of the loop of Henle. Although classically associated with renal sodium reabsorption and targeted by loop diuretics, NKCC2’s role in the brain is gaining attention. Emerging studies suggest that NKCC2 may influence cerebrospinal fluid composition, neuronal excitability, and osmoregulation. Dysregulation of NKCC2 could contribute to altered ion gradients and excitotoxicity, processes implicated in neurodegenerative diseases such as Alzheimer’s and epilepsy. Understanding NKCC2’s extrarenal functions may uncover novel mechanisms of ion homeostasis and therapeutic opportunities in neurodegeneration.
