HCN4 (Hyperpolarization-activated cyclic nucleotide-gated channel 4) is a key ion channel involved in generating rhythmic electrical activity in both cardiac and neuronal tissues. While best known for its role in cardiac pacemaking, HCN4 is also expressed in specific brain regions, including the thalamus and brainstem, where it contributes to the regulation of neuronal excitability and oscillatory activity. In the context of neurodegenerative diseases, HCN4 has gained attention for its role in maintaining rhythmic firing patterns and stabilizing neural circuits. Disruption of HCN4 function can lead to altered thalamocortical rhythms, which are implicated in cognitive dysfunction, sleep disturbances, and seizure susceptibility—common features in disorders such as Alzheimer’s disease and epilepsy. HCN4 channels are modulated by cyclic nucleotides like cAMP, linking them to intracellular signaling pathways that are often dysregulated in neurodegeneration. Their ability to influence synaptic integration and neuronal responsiveness positions HCN4 as a potential therapeutic target for restoring network stability in degenerative conditions.
