The vesicular acetylcholine transporter (VAChT), encoded by the SLC18A3 gene, is an essential membrane protein responsible for packaging acetylcholine (ACh) into synaptic vesicles for release at cholinergic synapses. By regulating the availability of ACh in the synaptic cleft, VAChT plays a central role in modulating cognitive function, motor control, and autonomic nervous system activity. In neurodegenerative diseases such as Alzheimer’s disease (AD), Parkinson’s disease (PD), and amyotrophic lateral sclerosis (ALS), cholinergic dysfunction is a well-established pathological feature. Reduced VAChT expression or impaired transporter function has been linked to synaptic failure, cognitive decline, and neuroinflammation. In AD, for example, VAChT levels correlate with disease severity and are considered a potential biomarker for cholinergic integrity. Recent advances in molecular imaging and transgenic models have enabled precise tracking of VAChT expression in vivo, offering new insights into the progression of neurodegenerative disorders. Moreover, VAChT is emerging as a promising therapeutic target for restoring cholinergic tone and improving cognitive outcomes. As a critical component of the cholinergic system, VAChT continues to be a focal point in neuroscience research, with implications for early diagnosis, disease monitoring, and the development of targeted interventions in neurodegenerative disease.
