HSP47, also known as SERPINH1, is a unique member of the serine protease inhibitor (serpin) superfamily that functions as a collagen-specific molecular chaperone. Localized in the endoplasmic reticulum, HSP47 plays a critical role in the intracellular folding, assembly, and secretion of procollagens—particularly type I collagen, a key structural component in tissue repair and remodeling. Unlike other heat shock proteins, HSP47 binds a single substrate: collagen. This specificity makes it a promising biomarker and therapeutic target in fibrotic diseases, where its expression is markedly elevated around fibrotic lesions. Recent studies suggest that modulating HSP47 activity could support cell-based therapies aimed at halting or reversing fibrosis progression. In the context of neuroscience, HSP47 is gaining attention for its role in post-injury tissue remodeling. Type I collagen is essential for scar formation and structural integrity following events such as myocardial infarction or central nervous system injury. HSP47 facilitates the proper positioning of collagen-producing cells and the assembly of collagen fibrils—processes that are also relevant in neurodegenerative conditions involving extracellular matrix dysregulation. Given its substrate specificity and stress-inducible nature, HSP47 represents a novel intersection between fibrosis, wound healing, and neurodegeneration, with potential as a universal marker and therapeutic target in regenerative medicine.
